The printed membership form

Somebody who wants to join at the bar on a Tuesday is handed a sheet, fills it in with a biro and gives it back with the money. That is still how most people join a bowls club, and no amount of online form replaces it — so Pavilion prints your club’s own application form, two sides of A4, ready to photocopy.

There is a Join form button on the Duty Steward toolbar, beside the rink sheet, so whoever is on the desk can produce one for somebody asking about joining.

📄 Nothing on it is typed in

The club name, the membership types, the fees and the contact for questions all come from your own settings. A form with prices written into it is wrong the day after the committee changes them — and every club would need its own copy of it.

💷 The joining offer appears only when there is one

The first side lists each membership and its annual fee. Where a joining offer is running, it also shows what somebody joining today would pay, and the form gives them a box for each. It asks Pavilion the same question the join page and Add Member ask, so the paper and the screen can never quote different figures.

Print it in August with an August–September offer set and the panel is there; print the same form in May and it is not, because a form offering a part-year price that is not available is worse than one that never mentioned it.

✍️ What it asks for

Title, name, address, telephone and mobile, email, date of birth, an emergency contact and their relationship to the member, which membership they want, the consent ticks for the directory, photographs and Gift Aid, whether they have bowled before, and a signature. At the foot there is a box for the club to record who took the payment and how.

Enter them on Add Member as usual. Pavilion works out the fee from the joining date, which is the figure already printed on the form they filled in.

Common questions

Can we change the wording?
The club-specific parts come from your settings — the name, the types, the fees, who to contact. The standard paragraphs are the same for every club, because they cover data protection and consent and are worded carefully on purpose.
Each membership shows its name and price but no description.
The first side reads a short description per membership type from your settings. Until those are written it prints the name and the fee alone, which is still a usable form.
Can a member fill it in on the tablet instead?
Use the online join form for that — the [pvl_join] page — which asks the same questions and lands the answers straight in Applications. The printed form is for the person who would rather have paper.
Does it cost anything to print?
It is an ordinary PDF. Print one and photocopy it, or print as many as you need — it is produced fresh each time, so it always has today’s prices on it.
